What Conditions are considered Chronic diseases? Chronic diseases are considered a medical condition lasting at least one year. These conditions require ongoing medical management. Examples of chronic diseases include Hypertension (High Blood Pressure), Heart disease, Asthma, Diabetes, and Obesity.
