WHAT IS CHRONIC DISEASE MANAGEMENT?

What Conditions are considered Chronic diseases? Chronic diseases are considered a medical condition lasting at least one year. These conditions require ongoing medical management. Examples of chronic diseases include Hypertension (High Blood Pressure), Heart disease, Asthma, Diabetes, and Obesity.

How are Chronic conditions treated? Chronic conditions need treatment and ongoing medical management. At Madison Integrative Medicine we work alongside you to manage your chronic condition and make an individualized plan of care for you.

What are causes of Chronic disease? There is not one single contributing factor to chronic conditions. Some risk factors that put you at risk of being diagnosed with one are:

  • Poor diet and nutrition

  • Lack of physical activity

  • Smoking and excessive alcohol use

  • Secondhand smoke

  • Obesity
